Course Archive Request

The Course Archive Request Form (MS Word or PDF) is used when a course is no longer taught AND to bring a course back that was previously taught on our campus.  Archiving a course does NOT delete it -- it can be reinstated at any time. 

Instructions for completing the form:

  1. Basic Course Information:
    College:  Trade Tech
    Subject code -- see list (MS Excel)
    Course number
    Course title -- as found in catalog
  2. Archive Requested:
    Check off the appropriate box.
  3. Effective semester -- allow 4 months processing time
  4. Approvals:  
    Signed by the department chair and dean of academic affairs

Submit the completed form to the Agenda Review Subcommittee to place the request on the next Curriculum Committee agenda.  If you are reinstating a course, you must also submit an updated course outline.  After the Curriculum Committee approves it, it will then go to the district for further approval.
